IMPORTANT THINGS TO REMEMBER WHEN COMPLETING THIS APPLICATION

• Remember to sign your child’s application!!! It is the parent’s/guardian’s responsibility to ensure that both the application and recommendation are in our office by January 31, 2008.
• Read all of the application carefully and fill out everything that is requested. Incomplete applications will NOT be accepted.
• Students applying from private, out of county, or home school MUST provide copies of the most recent standardized test results in math and reading as well as end of the year grades from last year and current grades for this year.
• Students must select one language strand, French or Spanish. (See the Recommendation and Language Selection Form)
• Applicants will be advised of application outcomes on or about March 15, 2008. For applicants receiving an offer of acceptance, parents/guardians must accept or decline the admission offer by March 31, 2008. Failure to respond by March 31, 2008 will be considered as a parental/guardian decline.
• The address on the application must match the one on record with Miami Dade County Public Schools.
• ALL APPLICATIONS ARE DUE JANUARY 31, 2008. PLEASE INCLUDE A STAMPED, SELF-ADDRESSED ENVELOPE.
• If you have another child currently in 6th or 7th grade at Ammons, be sure to answer the sibling question on the application.
• If your child has been in an advanced or intensive language program (French or Spanish), please write the following information at the TOP of the application page: the language, the name of the school, and the years enrolled.

DATOS IMPORTANTES ANTES DE LLENAR ESTA PLANILLA

• Recuerde firmar esta planilla. ¡¡FAVOR DE FIRMAR LA PRIMERA PÁGINA!! Es la responsabilidad del padre o del encargado del niño de asegurarse de que la planilla y la recomendación estén en nuestra oficina el día 31 de enero de 2008.
• Favor de leer cuidadosamente la planilla y completar todos los encasillados. NO ACEPTAREMOS PLANILLAS INCOMPLETAS.
• Los estudiantes provenientes de escuelas privadas, escuelas fuera del condado, o que reciben enseñanza en la casa, deberán presentar copias de los resultados del examen más reciente ("standardized test") de matemáticas y lectura. También deberán traer las notas de final de curso del año anterior y las de este curso escolar.
• Los estudiantes deben escoger uno de los idiomas, francés o español.
• Los solicitantes serán notificados de los resultados antes del 15 de marzo de 2008. Si usted recibe una oferta de aceptación, deberá de notificar si acepta la oferta o la rechaza antes del 31 de marzo de 2008. Si no responde antes del 31 de marzo de 2008, se considerará que usted a rechazado la propuesta.
• La dirección debe ser la misma que está en la computadora.
• Ú LTIMO DÍA PARA SOLICITAR ES EL 31 DE ENERO DE 2008. POR FAVOR ADJUNTE UN SOBRE SELLADO, DIRIGIDO A SÍ MISMO.
• Si el solicitante tiene actualmente un/a hermano/a en el sexto o séptimo grado en Ammons, favor de comunicar eso en la planilla.
• Si su hijo/a ha estado en un programa avanzado en francés o español, favor de escribir la siguiente información en la parte de arriba de la planilla: el idioma, el nombre de la escuela y los años que cursó.
